It is a flat hook with dolphines that I suppose will hang to a belt.

The crown on an M could mention that it is from Belgium city of Antwerpen year 1738 but I am not 100 % sure.

Thanks Myriam for the info, I thought that since I am from Malta it could also be M for Malta. I also do not know :-))
