Hoping there may be a member from Portugal who can shed some light on this medallion ( I can find no others on the internet)

My question is why it was made and what sort of age does it have?
It is marked on the edge 95% S (another possible letter after the S but to difficult to determine)
Measures 74mm in diameter and weighs 91g
